Medical Forms

Forms require medical review
A physician can only complete a form when there is enough information to provide an accurate medical opinion. An appointment, examination, records review, or testing may be required.
Bring every page of the form and complete the patient sections before the appointment.
Forms may include
- Driver medicals
- Employment forms
- School and camp forms
- Sports participation forms
- Insurance documentation
- Disability forms
- Workplace forms
- WorkSafeBC and ICBC documentation
Uninsured service and completion time
Many forms and reports are not covered by MSP and have a private fee based on complexity and time. Some cannot be completed during the appointment and may require additional processing time.
Frequently Asked Questions

Reception can advise, but the physician may require an appointment or additional information.
How long will completion take?
Timing depends on complexity, physician availability, records required, and whether an examination is needed.
Can the clinic guarantee approval?
No. The physician provides a medical assessment and cannot guarantee an employer, insurer, school, licensing body, or government decision.
