A physician can only complete a form when there is enough information to provide an accurate medical opinion. An appointment, examination, records review, or testing may be required.
Bring every page of the form and complete the patient sections before the appointment.

Uninsured service and completion time
Many forms and reports are not covered by MSP and have a private fee based on complexity and time. Some cannot be completed during the appointment and may require additional processing time.
